About the role

This is a full-time, hybrid position that requires a mixture of field work, office work, and time spent working remotely. Candidates must be based in South Los Angeles (SPA 6).

What You'll Do

• Working collaboratively with healthcare providers, hospitals, community organizations, and support networks, you will coordinate seamless, whole-person care that helps members achieve their goals.
• This role is highly community-focused, requiring you to meet members where they are by providing services in homes, shelters, clinics, and other accessible settings.
• You will:
• Conduct comprehensive assessments and develop personalized care plans that address the full spectrum of member needs, including physical health, behavioral health, substance use, housing stability, and access to social services.
• Advocate on behalf of members, connecting them to essential resources such as benefits, transportation, housing assistance, and community-based support.
• Use motivational interviewing, trauma-informed care, and harm-reduction practices to empower individuals to engage in their care, improve their well-being, and build long-term stability and independence.

What You'll Bring

• High school diploma or GED required; bachelor's degree in social work, psychology, counseling, nursing, or a related field preferred.
• Two or more years of experience supporting Medi-Cal members, behavioral health populations, or individuals with complex care needs.
• Strong relationship-building and communication skills with the ability to engage individuals from diverse backgrounds.
• Knowledge of community resources, care coordination, trauma-informed care, motivational interviewing, and harm-reduction practices.
• Current California driver's license, reliable transportation, and the ability to travel throughout the South Los Angeles community.
• Strong organizational, critical-thinking, and documentation skills with the ability to manage multiple priorities effectively.
