Lead Care Manager

About the role

The Lead Care Manager is a key member of a multidisciplinary team providing Enhanced Care Management (ECM) services. This role involves coordinating all aspects of ECM, overseeing service delivery, engaging members in ECM enrollment, and delivering ECM services.

Responsibilities

• Engage and enroll eligible members in ECM.
• Oversee the provision of ECM services and implementation of the care plan.
• Utilize motivational interviewing, trauma-informed care, and harm-reduction strategies.
• Coordinate services to ensure a seamless experience and avoid duplication of services.
• Connect members to other social services and support, including transportation.
• Provide community outreach and attend community events.
• Offer health promotion and self-management training.
• Maintain treatment adherence.
• Carefully coordinate with hospital staff on discharge plans.
• Implement program goals and objectives to meet and maintain contractual requirements and productivity.
• Prevent and control infections.
• Maintain a culture of quality and safety.
• Effectively use de-escalation skills and techniques for crisis prevention.
• Properly administer the C-SSRS to help prevent suicide.

Qualifications

• Bachelor of Arts or Bachelor of Science degree in Counseling, Social Work, or related field preferred or Associate of Science/Medical Assistant Certificate or Degree. Degree may be waived with equivalent work experience.
• Knowledge of community resources preferred.
• Proficient in Microsoft Office Suite, Outlook, Excel, Word.
• Bilingual required.
• Must be 21 years old or over.
• A valid California driver’s license is required of staff who have regular driving duties, and the driving record must meet the requirements of their position and be approved by the agency’s insurance company.
