About the role

Carmichael is an affluent suburb in the Greater Sacramento metropolitan region of Northern California and home to several breathtaking parks and beautiful gardens such as The Jensen Botanical Gardens; exhibitor of a great selection of amazing flora such as rhododendrons, dogwoods, camellias and azaleas. There is also The Chautauqua Playhouse; a theater which seats up to 95 people and shows a variety of musicals, comedies and dramas for both adults and children.

Responsibilities

• Provide clinical assessment, crisis intervention, care coordination, and discharge planning to help patients and families navigate medical, emotional, and social needs during hospitalization.

Requirements

• CA LCSWBLS5150 certified in CA
• 2 years recent acute hospital experience in specialty
• Must have prior Dignity/CommonSpirit experience to be considered
