Job Summary and Responsibilities

Position Overview: In this pivotal role, you will empower individuals grappling with mental illness to thrive within seamlessly integrated environments. Your work involves conducting face-to-face clinical assessments that are critical for admission into transitional adult homes. Assessments are conducted in nursing homes, hospitals, and community settings and are completed using the state's electronic system and internal systems that support determinations and notifications to applicable parties.

Job Responsibilities:

  • Demonstrate comprehensive knowledge of psychiatric disorders as outlined in the current Diagnostic and Statistical Manual used in New York.
  • Utilize expertise in standards of practice related to determining the appropriate level of care.
  • Apply an understanding of mental health disorders requiring long-term care services and support.
  • Exhibit familiarity with public sector services and support systems.
  • Utilize Microsoft Excel, Word, and Outlook for effective task management.
  • Assess and diagnose psychological and behavioral issues, delivering accurate clinical diagnoses.
  • Use computer equipment and web-based software proficiently for job-related tasks.
  • Collaborate with office staff to streamline workflows.
  • Communicate effectively with healthcare professionals, patients, families, and support networks.
  • Exhibit strong verbal, written, and electronic communication skills.
  • Exhibit attention to detail, task prioritization, and timely execution.
  • Analyze and evaluate psychological, emotional, behavioral, and physical functioning.
  • Develop recommendations regarding the appropriate level of care based on clinical evaluations.

Qualifications

  • Minimum of 2+ years of experience assessing individuals with mental and psychiatric disorders.
  • Must possess either an unrestricted active LCSW or RN license for New York State.
  • Must be a bona fide resident of the State of New York.
  • Hold a valid and current driving license in good standing in New York State.
  • Associate Degree or Bachelor’s Degree.
  • Proficiency in medical record abstraction, medical record organization, medical terminology, and disease processes.
  • Strong clinical assessment and critical thinking skills.
  • Exceptional verbal and written communication skills.
  • Ability to collaborate effectively within a team-oriented environment.
  • Strong organizational skills and flexibility.
  • Proficiency in Microsoft Office and internet/web navigation tools.
  • Two years' experience in a home care setting preferred.
