Responsibilities
- Complete comprehensive biopsychosocial assessments and/or psychiatric evaluations.
- Develop and maintain a treatment plan responsive to patient/family concerns, problems and clinical impressions.
- Use a patient and family centered approach to engage individuals and families in a plan of care that addresses their identified needs.
- Provide education and counseling to individuals and families.
- Utilize recognized best practices to provide individual, couples, family and/or group psychotherapy and/or crisis intervention as indicated.
- Consult and collaborate with external providers/agencies/organizations including but not limited to protective services, court systems, outpatient therapists, work/education settings and housing supports.
- Provide referrals to community-based services for individuals and families as indicated.
- Provide information, consultation and professional support to interdisciplinary team members and colleagues within the hospital and community.
- Facilitate and/or participate in interdisciplinary rounds and other patient related discussions to promote integrated care and improve patient outcomes.
- Demonstrate initiative and leadership in developing services and programs to enhance patient and family care, including development of evidence-based practices.
- Contribute to individual, team, departmental and organizational quality assurance and performance improvement.
- Document timely, accurate and clinically relevant entries of social work activities in the electronic medical record.
- Mentor/supervise and/or participate in projects/ work groups as assigned by supervisor.
- Navigate systems and provide advocacy for patients and families as indicated.
Qualifications
- Master's Degree from an accredited school of social work.
- Experience treating a broad spectrum of psychiatric conditions and working in a collaborative, multidisciplinary clinical setting.
- Training in cognitive-behavioral interventions.
- Ambition to develop program and services.
- Licensure (LCSW) to practice as a clinical social worker.
