LCSW - Home Health (PRN)

Responsibilities

• Guide and counsel individuals and families in crisis.
• Aid clients in obtaining government funds, education, or treatment.
• Evaluate medical, psychological, and social factors contributing to the client's situation.
• Converse with clients about personal and family issues, financial concerns, employment, housing, and health conditions.
• Evaluate the nature and extent of the client's problems and assess their capacities.
• Develop and implement service plans aimed at restoring patients to optimal social and health adjustment.
• Work independently while receiving support from the team.

Qualifications

• Master’s Degree in Social Work.
• Five plus years of clinical social work experience.
• Valid and current Licensed Clinical Social Worker or Licensed Medical Social Worker license.
• Ability to conduct psycho-social assessments and create care plans.
