About the Role
Spectrum Healthcare Resources has an opportunity for a civilian LCSW - Special Needs Coordinator (SNC) at Minot Air Force Base.
Schedule
- Outpatient Clinic
- Monday through Friday hours
- No night, No weekends, No On-Call
- 11 Paid Holidays

Requirements
- Sufficient knowledge to effectively apply Special Needs Coordination functions (Assessment, Planning, Facilitation & Advocacy)
- A minimum of two years full-time experience in care coordination, discharge planning or case management in a medical setting within the last four years
- Master of Social Work (M.S.W.) degree from a graduate school of social work fully accredited by the Council on Social Work Education (CSWE)
- LCSW License from any state
