Kidney Transplant Coordinator
About the role
As a Kidney Transplant Coordinator, you will oversee and manage all kidney transplant patient activities. In this role, you will assist fellow caregivers in creating care plans, reviewing medical information, assembling test results, coordinating transplants, and more, all to support the exceptional experience our community has come to expect. This role helps change the lives of patients and their families every day.
Responsibilities
- Assess patients in the outpatient clinic along with the transplant physician.
- Facilitate plans of care and symptom management with outpatient caregivers.
- Cover scheduling with the administrative coordinator for follow-up appointments.
- Review medical information from referring physicians and initiate appropriate communication.
- Coordinate consultations with other disciplines and obtain follow-up information.
- Coordinate discharge of transplant patients and donors.
- Absorb medications and supplies from the appropriate source according to the patient's insurance coverage.
- Contact referring physicians and arrange follow-up in the local area or at CCF depending upon patients and their activity.
- Prior to transplant, evaluate the patient for transplant candidacy, including facilitating tissue typing of recipient and donor, arranging pre-testing, and discussing with the transplant team.
- Assemble test results and arrange further testing if indicated.
- Serve as the primary contact for pre-transplant evaluation for physicians and/or insurance companies.
- Prepare and present cases to the Selection Committee.
- Place patients on the national waiting list for transplantation.
- Coordinate ongoing care of inpatients and outpatients waiting for transplant.
- Triage patients during the entire transplant process for any medical and psychosocial issues.
- Be available by beeper and by phone to provide immediate assessment and delivery of care in a timely and cost-effective manner.
- Collaborate with Office Coordinators and Financial Counselors in obtaining insurance clearance for patients prior to transplant procedure.
- Coordinate informational packets for patients prior to transplant.
- Review information with patients upon their arrival and answer any questions regarding the transplant information and transplant schedule.
- Coordinate and provide educational sessions for patients, families, and donors prior to transplant.
- Collaborate with the Organ Procurement Organization to facilitate organ procurement surgery and subsequent transplant surgery, including notifying all personnel involved in the process.
- Facilitate procedures.
- Provide in-services for nursing caregivers and presentations for any outside organization or group inquiring about the transplantation process.
Requirements
- Basic Life Support (BLS) Certification through the American Heart Association (AHA) or American Red Cross
- Two years of RN experience
- One year of acute care disease-related hospital experience
Preferred qualifications
- Bachelor of Science in Nursing (BSN)
- Transplant experience or GI experience
- ICU experience
- Endoscopy background
Physical Requirements
- Requires full range of body motion, manual finger dexterity, hand and eye coordination.
- Requires working under stressful conditions and working irregular hours.
- Must be willing to be on call 24 hours per day on a rotating basis.
