Job Posting Title Transitions of Care Pharmacist
United Health Services (UHS) is seeking a dedicated Pharmacist to join our Transitions of Care Pharmacy team. The role focuses on ensuring the safe and accurate administration of medications, verifying clinical appropriateness, and optimizing therapeutic outcomes. The Pharmacist will review complex medication regimens, address adherence barriers, and provide patient/caregiver education on medications and disease states.
Responsibilities
- Review medication orders for appropriateness (dose, route, frequency, indication, interactions, allergies)
- Perform medication reconciliation upon patient admission, transfer, and discharge
- Monitor patient therapy outcomes and adjust or recommend changes to optimize treatment
- Provide drug information to healthcare providers, patients, and caregivers
- Provide patient/caregiver education on medications and pertinent disease states
- Evaluate and prevent adverse drug events and medication errors
- Assess and address adherence barriers to medications prior to discharge
- Document all activities in the electronic health record (Epic)
- Ensure communication and coordination between healthcare providers and patient/caregivers
- Ensure compliance with state and federal regulations
Requirements
- A Bachelor’s of Science Degree (B.S.) or Pharm.D. in Pharmacy from an accredited college or university
- Licensed as a Pharmacist in New York State or NYS internship permit
Qualifications
Preferred: One year of experience as a pharmacist

$54.17 - $81.25 per hour, depending on experience
