About the role

Community Bridges is seeking a full-time Job Coach, Direct Support Professional for its Supported Employment Program in Concord, NH. This non-profit agency, founded in 1982, works to integrate people with developmental disabilities into their communities.

Responsibilities

  • Provide on-the-job assistance to individuals with employment barriers.
  • Assess vocational abilities and safety skills.
  • Coordinate career activities such as interviews, resume writing, and job placement.
  • Mentor and coach individuals to achieve success on career paths.
  • Support individuals on the job site, ensuring proper mentoring and success.
  • Create and implement fade programs and provide ongoing support.
  • Develop Individual Service Plans and provide necessary documentation.
  • Ensure confidentiality and respect for individuals' rights and dignity.
  • Attend team meetings, agency meetings, and trainings.
  • Perform delegated responsibilities, including filing, data collection, and other support tasks.

Requirements

  • High School Diploma or GED.
  • Experience in human service or related fields, with preference for coaching, mentoring, or vocational skills.
  • Reliable transportation, valid driver's license, and auto insurance.

Qualifications

  • Good interpersonal skills and the ability to initiate contacts with business representatives.
  • Effective teaching skills to facilitate job readiness.
  • Strong verbal and written communication skills.
  • Ability to advocate for and assist individuals in advocating for their rights.
  • Knowledge of Community Choices mission, values, policies, and procedures.
  • Computer competencies.

Skills

  • Ability to collaborate with families, team members, and community members.
  • First Aid, CPR, and MOAB certifications.

Schedule
