IRIS Consultant

We are seeking a compassionate and organized IRIS Consultant to support adults, older adults, and individuals with disabilities in Wisconsin's IRIS (Include, Respect, I Self-Direct) Program. This role involves case management, care coordination, social work, and community-based support to help participants maintain independence, achieve personal goals, and exercise self-direction. Key duties include building trusted relationships, developing and updating individualized support plans, conducting regular contacts and visits, monitoring health and safety, and collaborating with healthcare providers and community organizations. Qualifications include a Bachelor's degree in related fields with experience working with disabled or older adults, or extensive direct service experience. Preferred skills encompass care coordination, disability services, long-term care, and behavioral health. Bilingual abilities in languages such as Spanish, Hmong, Arabic, or ASL are highly valued. Candidates should possess strong communication skills, the ability to work independently, proficiency with technology, and a commitment to dignity and cultural awareness. A valid driver’s license, reliable transportation, and the ability to travel regionally are required.
