IRIS Consultant

Summary We are seeking a compassionate and organized IRIS Consultant to support adults, older adults, and individuals with disabilities through a person-centered, community-based program. Qualifications include a bachelor's degree in social services or related fields with at least one year of experience working with vulnerable populations, or equivalent direct experience. Preferred skills encompass care coordination, disability services, long-term care, and healthcare navigation. Bilingual abilities are highly valued. Essential skills include strong communication, cultural awareness, independence, and proficiency with technology. Valid driver's license and reliable transportation are required for travel within the community.
