IRIS Consultant

We are seeking a compassionate and organized IRIS Consultant to support adults and individuals with disabilities in promoting independence and self-direction through Wisconsin's IRIS Program. Key Responsibilities Include Building trusted relationships with participants and assisting in creating personalized IRIS Support and Services PlansConducting monthly contacts and quarterly in-person visits to monitor health, safety, and program outcomesPromoting participant choice, community inclusion, and self-determinationCollaborating with healthcare providers, community organizations, and resource centersMaintaining accurate documentation and assisting with assessmentsWorking independently within an interdisciplinary team to support participant goals Qualifications Include Bachelor’s degree in Social Work, Human Services, or related fields with at least one year of relevant experience, or four+ years of direct service experienceStrong communication, relationship-building skills, and cultural awarenessProficiency with technology and a valid driver’s license with reliable transportationAbility to serve diverse populations and work independently Preferred skills and experience: case management, care coordination, disability or long-term care services, behavioral health, and healthcare navigation.
