Internal Medicine Physician- Outpatient Community Health

About the role

Optimus Health Care is a leading provider of primary health care services in Fairfield County, with multiple locations in Bridgeport, Stratford, and Stamford. We are seeking an internal medicine physician interested in treating the general adult population and the older patient population, focusing on diagnosing and managing age-related conditions such as falls, confusion, chronic diseases, and coordinating care with interdisciplinary teams in a community health center setting.

Responsibilities

• Treat the general adult population and the older patient population
• Diagnose and manage age-related conditions such as falls, confusion, chronic diseases
• Coordinate complex care with interdisciplinary teams
• Practice in a multispecialty integrated care practice
• Work in a 100% outpatient setting

Requirements

• Medical Degree from an accredited Medical School
• Documented completion of a Residency from an accredited institution specializing in Internal Medicine
• Board Certified or Board Eligible to sit for Boards
• Valid License to Practice as an Internist in the State of CT
• Current DEA and State Controlled Substance Certificate
• CPR certification required upon hire and recertification bi-annually

Qualifications

• Ability to speak enough Spanish to examine Spanish-speaking patients and converse on a basic level (helpful)
• Professional, positive attitude, vision, understanding of customer service principles, trustworthiness, and excellent interpersonal skills
• Comfort level with medical complexity and multiple concurrent comorbidities
