Internal Director of Admissions

Aviata at San Jose is seeking an Internal Director of Admissions to support the admissions department's activities in pre-admission, admission, discharge, and follow-up of residents. The ideal candidate will conduct tours, review and coordinate patient acceptance, track referrals, and ensure proper approvals are obtained.

• Supports the admissions department's activities in pre-admission, admission, discharge, and follow-up of residents.
• Conducts tours of the facility for prospective residents.
• Reviews and coordinates with Liaison on patient acceptance.
• Tracks pre-admission referral activity daily and notifies departments of anticipated admissions.
• Affirms level of care approval and/or OBRA screen when necessary.
• Completes face sheets, attaches to clinical screens, copies, and distributes to relevant departments.
• Contact the County Department of Social Services/Medicaid office to verify eligibility and determine estimated co-pay/family participation amounts.
• Coordinates weekend/holiday admissions in conjunction with Liaisons.
• Initiates and completes specified components of the facility admission packet with patients/responsible parties.
• Maintains Resident Roster, waiting list, and inquiry list according to state regulations, including receipts.
• Coordinates clinical assessment/screening and ensures appropriate clinical acceptance in a timely manner in the absence of a Liaison.
• Provides innovative, responsible healthcare by creating and implementing new ideas and concepts to continuously improve systems and processes.

Qualifications

• Secondary Education Diploma or GED equivalency required.
• At least one (1) year experience in Health Care/ Business Development/ Insurance.
• Familiarity with medical terminology.
• Must be qualified, compassionate, and dedicated to a job well done.
