Intensive Support Professional-IRSH (PRN)

Overview

The Intensive Support Professional delivers hands-on intensive rehabilitation services to adults with mental health, behavioral health, intellectual disabilities, or co-occurring conditions. Key responsibilities include completing assessments, supporting individual goals, executing service plans, and managing medication.

Responsibilities

• Completes mental health and functional assessments; Identifies strengths and needs
• Aids individuals in exploring hopes, dreams, and goals
• Supports individual’s participation in service planning
• Executes individualized service plan centered around needs
• Engages in consultation and collaboration with additional support resources
• Creates and executes crisis plans
• Provides support and interventions for crisis de-escalation
• Trains in crisis intervention techniques
• Provides education on available supports
• Assesses individual’s ability to self-administer medication
• Maintains and manages medications
• Reports therapeutic actions and side effects
• Trains in medication management
• Supports activities of daily living
• Provides transportation and instruction on daily living activities
• Connects individuals to community resources and leisure activities
• Supports discharge planning and transition
• Documents care, progress, and concerns
• Participates in program planning, evaluation, and quality improvement
• Attends and participates in staff meetings and trainings
• Provides on-call duties as assigned

Qualifications

• High School Diploma
• One year experience in Behavioral or Mental Health
• Understanding and advocacy of individual’s rights
• Strong de-escalation and crisis management skills
• Ability to use usual and customary equipment
