About the role

The Integrated Health Therapist (IHC) at Sanford Health's Wahpeton Clinic serves patients as an integral member of a Patient-Centered Medical Home (PCMH) or Primary Care Clinic (PCC) team. They provide behavioral health services in a primary care setting, including diagnostic assessment, crisis intervention, and ongoing care coordination.

Responsibilities

  • Conducts diagnostic assessments and onsite crisis interventions
  • Determines the need for ongoing care based on clinical ability and complexity
  • Makes appropriate referrals and provides educational services
  • Follows up with referrals to specialty care and ensures behavioral health management
  • Takes leadership in universal health screening for behavioral and chemical health conditions
  • Works collaboratively with other team members to ensure quality care
  • Delivers empirically validated, short-term behavioral health care services
  • Works closely with other PCMH or PCC team members to address psychosocial and cultural elements of health
  • Aids in the management of populations of patients and tracks costs and quality
  • Evaluates the quality and effectiveness of behavioral health care and participates in quality improvement activities
  • Documents care responsibly and ethically to promote safe, reliable patient-centered care
  • Participates in a population health management approach to health care

Requirements

  • Master's degree in Counseling, Social Work, Psychology, or related Behavioral Health Sciences
  • Licensed Professional Counselor (LPC), Licensed Professional Clinical Counselor (LPCC), Licensed Master Social Worker (LMSW), or Licensed Clinical Social Worker (LCSW)

Qualifications

  • Knowledge of human behavior and legal/ethical issues related to behavioral health
  • Familiarity with medical care and emergency services
  • Comfortable working with all potential age populations and patients with co-morbid medical conditions
  • Ability to communicate effectively and work collaboratively in an inter-professional environment
  • Skills in multiple counseling modalities and competency in providing short-term behavioral health care

Pay

Salary ranges for LPC/LMSW and LPCC/LCSW positions are $28.00-$41.50 and $31.00-$46.50 respectively. Sign On Bonus available!

Schedule

Full time, 8 hours per day, Monday through Friday.
