About the role
We are seeking an Integrated Medicine and Wellness Coordinator to join our team in the Dallas area. The position supports the strategic coordination of our integrated care model, focusing on enhancing Adult Medicine services.
Responsibilities
- Coordinate integration of Adult Medicine with Pharmacy, Behavioral Health, Dental, Nutrition, Vision, Imaging, Psychiatry, OB/GYN, and other ancillary services to improve coordinated patient care.
- Support Adult Medicine providers with internal referrals, care pathways, and navigation across multiple service lines.
- Maintain integration resources such as care coordination protocols, workflow maps, service directories, and orientation materials.
- Affiliate with department leaders to develop patient engagement strategies, educational resources, and multidisciplinary care initiatives that improve access and outcomes.
- Monitor operational metrics such as referral volumes, provider participation, and service utilization, while coordinating Community Health Workers and Educational Specialists.
- Facilitate onboarding and integration of new services, staff, and programs into Adult Medicine, ensuring alignment with HHM’s whole-person care model.
- Serve as a liaison for external partners and coordinate interdepartmental communication, meetings, training, documentation, reporting, and integrated care initiatives.
Requirements
- Bachelor’s degree in Healthcare Administration, Public Health, Social Work, nursing, or a related field preferred, though equivalent experience may be considered.
- Master’s degree in healthcare administration, Public Health, or Social work preferred.
- Minimum of four years of healthcare administrative experience, preferably supporting Adult Medicine, Internal Medicine, or related clinical services.
- Experience working within integrated or multidisciplinary care models, with a strong understanding of coordinated patient care.
- Familiarity with Federally Qualified Health Centers (FQHCs) and integrated care delivery systems.
- Knowledge of value-based care principles and experience supporting HEDIS, UDS, and adult health performance measures.
- Understanding of behavioral health integration, medication adherence programs, nutrition services, vision care, and other supportive services within primary care settings.
- Strong organizational, communication, and coordination skills with the ability to collaborate effectively across clinical and administrative teams.
- Proficiency in eClinicalWorks, Microsoft Office Suite, Teams, Excel, healthcare quality reporting, and managing multiple projects with strong attention to detail.
