Integrated Health Registered Nurse

Responsibilities

• Facilitate off-site and on-site educational services that focus on the influence of physical/mental health, the purpose and side effects of medications, and proper medication adherence.
• Communicate with clients and insurance companies, collect data from patients to assist in managing their care, and coordinate/collaborate with other medical providers for continuity of care.
• Request and review medical documentation from previous/current providers for continuity of care.
• Provide medical consultation with Comprehensive Assessments to determine readiness for Colbert/Williams members to transition into the community.
• Provide medical support with consumers in a natural environment including but not limited to home visits, doctor appointments, and nursing homes.
• Spend 60% of work time providing services off-site.

Qualifications

• Registered Nurse (RN) required.
• Minimum of a Bachelor's Degree in Nursing (BSN).
• Minimum of one year of professional RN experience.
• Valid driver's license and documentation of current auto insurance, with an acceptable motor vehicle report.
• Effective communication skills, both written and oral.
• Proficient in the use of computers, software applications, and working knowledge of Microsoft Office Suite programs.

Pay

Pay range: $70,000-80,000

Schedule

Spend 60% of work time providing services off-site.
