Integrated Health Coach (Fluent in Burmese) - Allen County, IN
Location: Across Allen County, IN — wherever members need you most! Rate depends on language proficiency (Burmese fluency at $25 /hr). Full‑time, in‑person, Monday – Friday 9 AM – 5 PM.
About the Role
Vanna Health is seeking a Coach passionate about supporting individuals with behavioral health challenges to lead fulfilling lives within their communities. Coaches locate and engage members, build trusting relationships, understand personal goals, and connect members to resources. They collaborate with clinicians and specialists to ensure high‑quality physical and behavioral health care, interacting in person, by phone, text, and email.
Responsibilities
- Engage and build trust: locate and engage Vanna members in the community and establish collaborative relationships with their care team.
- Achieve enrollment goals: meet daily member enrollment and engagement targets to support program growth.
- Support recovery and self‑determination: help members create personal action plans and track progress toward recovery goals.
- Role modeling and encouragement: act as a positive role model, providing consistent support at all stages of recovery.
- Feedback for continuous improvement: provide constructive feedback to Vanna technology and operations teams and suggest process improvements.
- Wellness and coping strategies: encourage and facilitate wellness strategies that enhance coping skills and support physical and behavioral health management.
- Physical health coaching (training provided): deliver physical health coaching as needed, following Vanna‑provided training.
About You
Required:
- Over two years of experience working directly with people diagnosed with serious mental illness, both in the field and in the community.
- Experience working in and around the communities of Allen County, IN.
- Valid driver’s license.
- High school diploma or equivalent.
- Fluent in the Burmese language.
Nice to Have:
- Spanish fluency or proficiency in other languages.
- Certified Peer Specialist training.
- Experience with substance‑use disorders.
