Job Summary
Central Health is seeking a skilled Licensed Master Social Worker or Licensed Clinical Social Worker to join our Behavioral Health Services team. The ideal candidate will provide compassionate, inclusive, and trauma-informed care, focusing on reducing systemic barriers to mental health and substance use support, treatment, and services. Responsibilities include crisis intervention, psychoeducation, clinical consultation, and group facilitation.
Job Functions
- Respond to mental health and substance use-related crises through trauma-informed crisis intervention, de-escalation, risk assessment, and evidence-informed interventions.
- Work closely with the patient’s interdisciplinary care team, provider, and other licensed healthcare facility providers to provide care that aligns with Central Health’s policies, mission, and goals.
- Aid the patient’s care team or facilitate patient-related meetings to establish care priorities, goals, and planning.
- Complete risk screenings and assessments of the patient’s multi-dimensional and holistic biopsychosocial-spiritual needs, strengths, and goals to guide treatment planning.
- Deliver effective evidence-informed and emerging best practice clinical treatments in individual and group settings.
- Provide strengths-based and culturally responsive psychoeducation to individuals and groups, including health literacy and harm reduction strategies.
- Manage a caseload of patients to support continuity of care.
- Advocate with internal and external stakeholders for the patient’s choices, values, and preferences, supporting safe and seamless continuity of care through care coordination services.
- Foster a safe, supportive, and healing environment for all patients and their support systems.
- Attend and participate in multidisciplinary team meetings, staff meetings, supervision sessions, trainings, and other relevant meetings.
- Complete all direct and indirect patient care documentation in accordance with professional standards, program procedures, and relevant local, state, and federal regulations.
- Maintain knowledge of community resources and other benefits to refer patients for services and advocate for their needs, priorities, and goals.
- Foster a collaborative and professional environment within the organization and with external community partners.
- Participate in relevant professional development training to ensure the delivery of best clinical practices and maintain all licensure and/or certification requirements.
- Perform other duties as assigned.
Minimum Education
A Master's Degree in Social Work from an accredited program.
Minimum Experience
A minimum of 1 year of clinical experience in mental healthcare.
PREFERRED EXPERIENCE
2 years of clinical experience in mental healthcare and substance use treatment.
Required Certifications/Licenses
- Active and unrestricted Licensed Master of Social Worker in the state of Texas (LMSW).
- Willingness to be under clinical supervision by a Licensed Clinical Social Worker Supervisor.
PREFERRED CERTIFICATIONS/LICENSURES
- Active and unrestricted LCSW in Texas.
