Responsibilities
- Provide brief, solution-focused interventions for patients of all ages during primary care visits as needed.
- Deliver short-term, evidence-based therapy within the primary care clinic to patients for whom extended outpatient care is not indicated.
- Conduct focused assessments and screenings to support diagnosis, treatment planning, and provider decision-making.
- Collaborate directly with primary care providers and team members to address behavioral health needs as they arise.
- Support patients and families in managing behavioral health concerns related to chronic conditions, stress, lifestyle changes, and psychosocial challenges.
- Facilitate warm hand-offs and referrals for patients requiring specialty behavioral health or community-based services.
- Participate in team huddles, case consultations, and clinic quality improvement efforts.
- Maintain accurate and timely documentation in compliance with organizational, regulatory, and payer standards.
- Engage in ongoing professional development, maintaining licensure and required continuing education.
Requirements
- Master’s degree or higher in Counseling, Social Work, Psychology, Marriage & Family Therapy, or a related behavioral health field.
- Active North Carolina license as LCMHC, LCSW, LMFT, LCAS, and/or LP (full license required; provisionally licensed applicants may be considered based on program needs).
- Demonstrated training and experience in evidence-based brief interventions (e.g., CBT, DBT, MI, solution-focused approaches).
- Strong communication and organizational skills; ability to thrive in a fast-paced, team-based setting.
Preferred
- Experience providing behavioral health services in a primary care or integrated setting.
- Familiarity with population health or collaborative care models.
- Experience delivering both brief interventions and short-term therapy to diverse populations.
