Intake Coordinator- Behavioral Health

Responsibilities

  • Responds to intake calls and completes admission paperwork.
  • Assessesments will be completed by RN, LPC or LMSW.
  • Facilitates groups, family and individual sessions and completing all necessary documentation.
  • Schedules Unit assessments, meetings and appointments as appropriate.
  • Utilizes assessment tools including, but not limited to Geriatric Depression Scale, Zung, SLUMS, alcohol screening tool(s) under authorization/supervision of RN, LPC or LMSW.
  • Communicates with referral sources including, family members, legal system, psychiatrists, primary care physicians, and other treatment facilities.
  • Maintains schedule for filing court paperwork and hearings. Works closely with court personnel to maintain good relationships.
  • Participates as an active member of the interdisciplinary team.
  • Maintains compliance with state and federal confidentiality laws as well as upholding our Horizon’s confidentiality policies and procedures.
  • Identifies areas needing improvement and utilizes the facility performance improvement process. Actively participates in department processes as requested.
  • Enters required data into HorizonPlus system and generates monthly reports to track and trend referral, admission and discharge data.
  • Assists in submission of reports and requests for documents from various regulatory agencies; re: PA Dept. of Health, CAP and Joint Commission.
  • Prepares and disseminates monthly reports and completes required follow up on activities on behalf of the Director.
  • Prepares and submits all operational tasks/documentation (census report, PIORS) as required by the corporate offices.

Qualifications

  • A Bachelor’s Degree from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing or other related field is preferred; and/or
  • A Master’s Degree or above from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing or other related field which includes a practicum in a health or human service agency is preferred.
  • Minimum three years’ of experience in a similar position and/or industry.
