Intake Coordinator

Under the direction of the Director of Counseling Services, the Access and Care Navigator serves as the primary point of entry for individuals seeking Network 9 Behavioral Health Services. This position responds to requests for services, provides same-day engagement whenever possible, completes consents, screenings, assessments, and referrals, and assists individuals in accessing the most appropriate behavioral health services.

Reports To: Director of Counseling and Specialty Services

Responsibilities

  • Respond to requests for behavioral health services by conducting initial screenings and determining appropriate service needs.
  • Meet with individuals seeking services as quickly as possible to complete consents, screenings, assessments, and other required intake documentation.
  • Coordinate referrals and appointments across all Network 9 programs and service locations to ensure timely access to behavioral health services.
  • Manage centralized wait lists by monitoring program capacity, communicating with program staff, and facilitating placement into services as openings become available.
  • Collaborate with clinicians, program staff, referral sources, healthcare providers, schools, hospitals, and community agencies to coordinate admissions and continuity of care.
  • Educate individuals served and family members regarding available services, eligibility requirements, insurance coverage, and treatment options.
  • Maintain accurate and timely documentation in accordance with organizational policies, Medicaid requirements, accreditation standards, and applicable regulations.
  • Participate in quality improvement activities designed to improve access to care, reduce wait times, and enhance the overall experience of individuals seeking behavioral health services.

Requirements

  • Bachelor's degree in Counseling, Psychology, Social Work, Human Services, or a related behavioral health field required.
  • Minimum of two years of experience in behavioral health, care coordination, intake, case management, crisis intervention, or a related human services setting preferred.
  • Experience conducting behavioral health screenings, coordinating referrals, or navigating individuals through behavioral health systems preferred.
  • Experience working with electronic health records, scheduling systems, Medicaid, and commercial insurance preferred.
  • Knowledge of community behavioral health resources and referral processes.
  • Ability to establish and maintain effective working relationships with individuals served, families, referral sources, healthcare providers, and community partners.
  • Valid Illinois driver's license, reliable transportation, and proof of automobile insurance required.

Skills

  • Knowledge of behavioral health assessment, screening, intake, care navigation, and referral processes.
  • Knowledge of outpatient behavioral health services, crisis intervention, community resources, and levels of care.
  • Excellent customer service, interpersonal, and communication skills while creating a welcoming and person-centered experience for individuals seeking services.
  • Ability to establish rapport and effectively engage individuals, families, referral sources, and community partners.
  • Strong organizational and time management skills with the ability to manage multiple referrals, assessments, appointments, and competing priorities.
  • Ability to assess needs, exercise sound clinical judgment, and facilitate timely access to appropriate behavioral health services.
  • Knowledge of Medicaid, commercial insurance, confidentiality requirements, and behavioral health documentation standards.
  • Proficiency with electronic health records, scheduling systems, Microsoft Office applications, and other technology used to coordinate care.
  • Ability to collaborate effectively with multidisciplinary teams to support continuity of care and positive outcomes.
  • Demonstrates commitment to person-centered, trauma-informed, culturally responsive, recovery-oriented, and strengths-based practices.

Physical Demands

  • Remains in a stationary position, including sitting or standing, for prolonged periods.
  • Operates a computer, telephone, and other standard office equipment.
  • Frequently communicates with individuals served, family members, referral sources, employees, and community partners in person, by telephone, and through virtual platforms.
  • Occasionally travels between Trinity Services locations and community partner sites as needed.
  • Operates a motor vehicle while conducting agency business.
  • Occasionally lifts and carries materials weighing up to 25 pounds.
  • Occasionally reaches, bends, stoops, kneels, and performs repetitive hand and wrist movements associated with computer use.

Pay

$45,000-$47,500
