About Cascade

Cascade Community Healthcare (CCH) has been providing treatment services to Lewis County and surrounding areas for more than 50 years. Our mission is to provide our clients with compassionate and encouraging counseling leading to a renewed sense of hope and the ability to make positive changes and live more satisfying lives. Cascade currently has six treatment locations in Lewis County.

Responsibilities

  • Provides intake assessment, primary same-day walk-in assessments, and treatment planning.
  • Completes intake assessments within timeframe set by the agency.
  • Completes treatment plans within timeframe set forth by the agency.
  • Reviews treatment plans goals and short-term objectives with client at each individual treatment session, tracks progress towards meeting those goals, and records client responses in client’s own words in the clinical record.
  • If applicable, provides individual, family, and case management services to assigned clients as appropriate.
    • Meets with client in accordance with expectations outlined in the treatment plan.
    • Ensures proper level of service by reviewing client needs and adjusting treatment plan accordingly to maintain proper utilization of services as evidenced by over/under utilization reports.
  • Meets direct service productivity requirements as assigned by the program manager/director.
  • Coordinates treatment services with schools, other relevant agencies, and significant others.
    • Outlines significance of working with primary care providers and other relevant agencies and significant others at intake or time of development of the treatment plan evidenced by treatment plans and progress notes and releases of information.
    • If applicable, periodically encourages client to include the above at various points of treatment when medically necessary or appropriate as evidenced in treatment plans and progress notes.
  • Maintains records and reports in accordance with agency policies and procedures.
    • Completes all required documentation and data verification to be available to MIS according to agency policy and procedure.
    • If applicable, completes treatment plans, crisis plans, and progress notes according to agency policy and procedure.
    • Completes other forms as required by agency policy and procedure.
  • Participates in supervision, consultation, and training at the direction of the Program supervisor.
    • Meets weekly with Program Manager for supervision and to discuss support staff needs.
    • Attends program-specific staff meetings.
    • Completes all trainings as assigned within the allotted timeframe set forth by the agency.
    • Participates in quality improvement activities as outlined by the agency.
  • Coordinates client data processing procedures.
    • Produces required reports and paperwork within timeframe set forth by the agency.
    • Arranges schedule to allow time for coordination of care and return phone calls within one business day (i.e., answer questions for staff related to client data, communication to support integrated care, follow up with client/family, etc.)

Requirements

  • Master’s Degree in counseling or the social sciences required.
  • Must be Licensed (LMHC, LMFT, LICSW) or have Associate License credential (LMHCA, LMFTA, or LICSW).
  • Must be able to pass WA State Patrol background check. A national background check may be required if you have not lived in WA state consecutively the last three years.
  • Must comply with agency policies and procedures.
  • Must have a valid driver's license.
  • Must be able to operate a motor vehicle.
  • Must be able to gain access to all types of buildings and residences regardless of the structures’ handicapped accessibility or lack thereof.
  • Must maintain current phone number and mailing address on file with the Human Resources office.

Pay

  • Licensed: $34.85 - $42.48 Hourly
  • Associate License: $31.78 - $38.74 Hourly

Schedule

Day Shift: Monday - Friday, 40 hours weekly

  • Cascade is an eligible employer for the Public Service Loan Forgiveness (PSLF) Program.
  • Paid time off: Start off with 13 hours of paid time off (PTO) earned monthly, and 11 paid holidays plus 16 hours of personal holiday. Pro-rated based on FTE. PTO and Personal Holiday are available after 90-days of employment.
  • Company Paid Employee Benefits: Medical, Dental, Vision, AllyHealth, EAP, Life insurance and AD&D, Long Term Disability, and Supplemental Benefits (Critical Illness, Hospital Indemnity, and Accident Insurance), subject to plan terms. Medical benefits will be effective the first of the month following date of hire. (0.5FTE not eligible for Medical, eligible for all other benefits).
  • Voluntary Benefits: Supplemental Term Life Insurance, Supplemental Legal Care, Fraud & Identity Protection, HSA, FSA, Dependent Care FSA, Limited Purpose FSA, and Pet insurance, subject to plan terms.
  • Retirement: 401(k), after 6-months of employment Cascade contributes 5% of your gross earnings monthly. No match required.
  • Cascade reimburses the cost of Department of Health licensure renewals and other certification renewals that are required for you to do your job (DEA, CPR/BLS, etc.).
