About the role

As an Intact Case Manager, you will be responsible for providing comprehensive support to individuals navigating complex healthcare systems. This includes coordinating care, managing case files, and ensuring timely and effective service delivery.

Responsibilities

• Coordinate care plans and ensure seamless transitions between providers and services.
• Manage case files and maintain accurate records of client interactions and progress.
• Communicate regularly with clients to monitor their health status and needs.
• Collaborate with interdisciplinary teams to develop and implement care strategies.
• Provide educational resources to clients and families to enhance understanding of their health conditions.

Requirements

• Bachelor’s degree in a related field such as healthcare, social work, or public health.
• At least 2 years of relevant experience in healthcare administration or case management.
• Strong organizational and interpersonal skills.
• Proficiency in Microsoft Office Suite.
• Ability to work independently and manage multiple tasks simultaneously.

Qualifications
