Inpatient Pharmacist

About the Role

Under indirect supervision, this role involves consulting with patients and medical personnel regarding medication therapy, dispensing compounds, procuring, storing, and distributing pharmacy products, and providing medication-related information and product identification.

Responsibilities

• Monitors and/or manages the medication therapy of specific hospitalized or ambulatory patients to assure optimum therapeutic results and cost-effective prescribing.
• Reviews laboratory test results and patient progress to detect adverse drug reactions; confers with and assists medical personnel in laboratory test interpretation, care, and treatment of patients.
• Interviews patients to obtain information regarding medication use, allergies, and sensitivities; documents this information in electronic medical records.
• Advises patients on directions for use, medication storage, compliance importance, precautions, and warnings for medication therapy.
• Provides medical personnel with medication information, drug therapy options, and pharmacokinetic dosing advice.
• Orders and adjusts dosages, orders laboratory tests, and administers medication under protocol according to established policies and procedures.
• Dispenses, compounds, procures, stores, and distributes pharmacy products; monitors the effectiveness and accuracy of medication distribution.
• Reviews physician orders and enters them into the patient’s electronic medical record; verifies accuracy of transcription or data entry.
• Verifies accuracy of medication preparation and dispensing by pharmacy personnel.
• Reviews patients' medical records to verify proper delivery and correct administration of medication; reports discrepancies to appropriate personnel.
• Participates in assigned education and training programs; provides in-services and educational programs for pharmacy and other medical personnel, as well as patients.
• Participates in patient-care rounds, hospital committees, utilization review, and quality management programs.
• Provides direction to Pharmacist Residents, Pharmacist Interns, Pharmacy Technicians, and other non-licensed personnel.
• Performs other duties as required.

Requirements

• B.S. degree in Pharmacy OR Pharm.D required.
• Pharmacist License (California) within 1 month of hire.
• Basic Life Support certification.
• National Provider Identifier required at hire.
• Comprehensive knowledge of current pharmacological/biopharmaceutical principles, medical terminology, pathologies, disease states, and related information for a medical management plan.
• Knowledge of aseptic technique and the preparation of sterile IV products and chemotherapeutic agents.
• Knowledge of emergency medications and drug monitoring.
• Excellent verbal and written communication skills.
• Ability to operate pharmacy computer systems and other equipment, and generate prescription labels.
• Must pass a background check.
• Ability to work in a Labor/Management Partnership environment.

Schedule

• Part-time, 24 scheduled weekly hours.
• Variable shift: Sunday-Saturday, may include weekends, days, evenings, and nights.
• Working hours: 12:01 AM – 11:59 PM.

Pay

$109.14 - $112.41 per hour. Kaiser Permanente strives to offer a market-competitive total rewards package and is committed to pay equity and transparency. The posted pay range is based on possible base salaries for the role.
