Responsibilities
- Clinical Vignette & Order Management: Review, validate, and optimize medication orders for clinical appropriateness. Proactively screen for drug interactions, therapeutic duplications, and contraindications, partnering directly with prescribers to resolve complexities and meticulously document clinical interventions.
- Advanced Compounding & Precision Dispensing: Oversee and accurately compound specialized sterile and non-sterile medications—including high-stakes chemotherapy and Total Parenteral Nutrition (TPN)—strictly adhering to regulatory standards to ensure flawless dispensing, labeling, and inventory control.
- Interdisciplinary Collaboration & Safe Transitions: Serve as the pharmacy voice during patient rounds and clinical committees. Design tailored, patient-specific medication regimens and lead the medication reconciliation process during admissions and discharges to eliminate errors and secure seamless transitions of care.
- Patient Consultation & Complex Care Advocacy: Conduct focused, high-touch consultations for patients on complex or high-risk regimens (such as anticoagulants or insulin therapies), empowering them with clear education on administration, side effects, and adherence to optimize their therapeutic outcomes.
- Team Leadership & Operational Mentorship: Supervise and elevate pharmacy technicians, ensuring rigorous quality control and unwavering regulatory compliance. Serve as a mentor to pharmacy students and residents while managing workflows across central, satellite, or specialty pharmacy services.
Requirements
- Pharmacist credentialed from the Wisconsin Pharmacy Examining Board obtained prior to hire date or job transfer date required.
- Bachelor's or advanced degree in Pharmacy required.
