Job Summary

The Social Worker provides comprehensive psychosocial assessments to members. Services aim to support and maximize members' ability to direct their own health care experience through assessment, intervention, educational programs, and resource linkage.

About the Role

Social work contributes to the medical teams' understanding of patient beliefs, behaviors, and barriers regarding illness. Encourages respect for individuality, independence, and patient choice while collaborating on policies and systems respecting cultural, religious, and ethnic differences.

Responsibilities

• Uses age-appropriate assessment and intervention skills.
• Able to use a range of interventions based on patient/family needs.
• Aids members in creating cost-effective, appropriate health care plans by optimizing personal resources and Kaiser Permanente and community programs.
• Promotes multi-disciplinary care that is patient-centered and considers all aspects of personal, psychological, economic, and cultural needs.
• Maintains thorough knowledge of Kaiser Permanente services/benefits, public/private community resources, and relevant laws.
• Completes required documentation and Social Work plan of care according to department policy.
• Participates in program development, orientation, and educational activities enhancing members' health care self-direction.

Requirements

• Minimum one (1) year of Social Work experience in a hospital, medical outpatient clinic, home health/hospice, or long-term care setting -OR- successful completion of the Kaiser Permanente NW Social Worker Internship program.
• Minimum three (3) years of additional experience in a related environment.
• Masters degree in social work from an accredited university.
• Credentials from the primary work state required at hire.
• Additional Credentials from the secondary work state(s) required post-hire.
• Ability to work effectively on a multi-disciplinary team.

Qualifications

• Basic Qualifications: One (1) year of Social Work experience in a hospital, medical outpatient clinic, home health/hospice, or long-term care setting -OR- successful completion of the Kaiser Permanente NW Social Worker Internship program.
• Three (3) years of additional experience in a related environment.
• Education: Masters degree in social work from an accredited university.
• Licenses/Certifications: Required at hire - Clinical Social Work Associate Certificate (Oregon), Licensed Clinical Social Worker (Oregon), Masters Social Worker License (Oregon); Licensed Social Worker Associate-Independent Clinical (Washington), Certified Social Worker Associate License (Washington), Licensed Advanced Social Worker License (Washington), Licensed Independent Clinical Social Worker (Washington).
• Additional Requirements: Ability to work effectively on a multi-disciplinary team.
