INPATIENT MATERNAL CHILD CASE MANAGER- LMSW or LCSW
About the role
A Licensed Social Worker (LMSW or LCSW) with experience in a hospital setting, preferably supporting maternal-child health, pediatrics, women’s health, and families. The role focuses on psychosocial assessment, care coordination, discharge planning, patient advocacy, health education, and interdisciplinary collaboration.
Key Responsibilities
- Conduct comprehensive psychosocial assessments of maternal, pediatric, and family populations to identify barriers to care, provide crisis intervention, and connect patients with appropriate community and healthcare resources.

- Coordinate patient care across the continuum by collaborating with physicians, nurses, and interdisciplinary team members to develop, implement, and evaluate individualized care and discharge plans.
- Educate and support patients and families on prenatal care, infant care, safe sleep practices, postpartum mental health, and available support services while advocating for resources that promote positive health outcomes.
Requirements
- Education: Master's in Social Work required
- Licensure: LMSW or LCSW/LCSW-C required
- Certification: ACM, CCM preferred
- Experience: 2-4 years of social work experience in a hospital setting with maternal child health, children and families is preferred
