About the role
This pharmacist will practice within a progressive clinical pharmacy model at Upstate Community Hospital, joining a talented team to deliver exceptional pharmaceutical care. The role focuses on providing care and oversight for safe and efficient transitions of care among patients admitted and discharged from the hospital. As an academic medical center, opportunities exist for developing pharmacy educational programs and participation in research projects. Pharmacists at Upstate have the opportunity to practice at the top of their pharmacy licensure.
Responsibilities
- Interpret medication orders and prescriptions to ensure they are complete, safe, rational, and compatible with institutional policy.
- Prepare compounds and dispense medications for inpatient and ambulatory care use, including injectable sterile products and non-sterile compounds.
- Perform duties in various practice locations, including inpatient, emergency departments, ambulatory care clinics, and transitions of care settings.
- Provide information to healthcare practitioners to promote the safe and rational use of medications.
- Provide drug monitoring services and maintain pharmacy records according to law and institutional policy.
- Interpret policy and procedures for hospital staff.
- Participate in teaching programs for the department and hospital staff.
- Participate in the department’s quality assurance programs.
Requirements
- B.S. in Pharmacy or Pharm.D.
- NYS Pharmacist license and registration (or eligible). Candidates who possess a NYS pharmacist limited permit must obtain license within 1 year of appointment.
Preferred Qualifications
- Previous hospital experience preferred.
Schedule
Rotating shifts, including some weekends and holidays.
