Inpatient Clinical Pharmacist

Responsibilities

• Provide direct clinical pharmacy services to hospitalized patients, ensuring safe, accurate, and timely medication use.
• Verify and dispense medication orders accurately.
• Perform clinical pharmacotherapy reviews and monitor drug therapy and lab results.
• Collaborate with physicians and nurses during rounds to optimize patient care.
• Educate patients on their medications to promote understanding and adherence.
• Support antimicrobial stewardship and medication safety initiatives.
• Document interventions and clinical activities in the electronic health record.
