Inpatient Case Manager/ Registered Nurse (RN)- Full Time Weekend Dayshift

About the role

Sentara Norfolk General Hospital is hiring an Inpatient Case Manager/Registered Nurse (RN) on a full-time weekend day shift. This position is eligible for up to a $20,000 sign-on bonus for qualified candidates.

Schedule

• 3×12 weekend schedule (36 hours): 8:00 AM to 8:30 PM EST, Friday, Saturday, and Sunday OR Saturday, Sunday, and Monday, including a 30-minute lunch break
• 4×10 weekend schedule (40 hours): 8:00 AM to 6:30 PM EST, Friday through Monday, including a 30-minute lunch break
• Eligible for weekend shift differential
• Training Monday through Friday from 8:00 AM – 4:30 PM for the first several weeks before transitioning to the full-time weekend day shift

Responsibilities

• Provide and facilitate comprehensive care management, services, and quality outcomes for patients across the healthcare continuum
• Primary focus on patient transition to the appropriate level of care while facilitating clinical appropriateness and length of stay
• Directly support in-hospital patients identified as needing comprehensive case management
• Promote effective utilization and monitoring of health services
• Collaborate and communicate with the healthcare team and patient/caregiver to manage care and transitions
• Develop and/or implement a comprehensive care plan based on assessment and evaluation to include physical, mental, and psychosocial needs of the inpatient/caregiver
• Function in a hospital practice setting

Requirements

• Bachelor’s Degree in Nursing preferred
• Registered Nurse (RN) license in Virginia, North Carolina, or a Compact/Multi-state license required
• BLS within 90 days of hire
• Case Management certification (ACM, CCM, CCCTM, CMAC, or CMGT-BC) required within one year of eligibility
• 1 year of nursing experience
• 1 year of acute care case management preferred
