Job Description
Captures the total Plan of Care for the patient in the Home Care setting, assessing, planning, implementing, and evaluating.
Education And Credentials
- Associates Degree required.
- Graduate of accredited School of Registered Nursing.
- Current licensure to practice as a Registered Nurse in NYS.
- Valid NYS drivers license.
Experience
- 1-5 years of relevant experience.
- One (1) year medical/surgical experience as a RN.
- One year infusion experience.

- Department: VNA IV # 1
- Standard Hours: Bi-Weekly 80.25
- Weekend/Holiday Requirement: Yes
- On Call Required: Yes With Rotation
- Scheduled Work Hours: 8a-4:30p
- Work Arrangement: Onsite
