Infusion Staff Pharmacist

Scope of Position

• Verifies new medication orders
• Checks prepared medications
• Oversight of dispensing from the pharmacy
• Completes clinical patient review for safety and accuracy
• Provides safe, efficient, high quality, patient-centered, and evidence-based medication services
• Acts as a resource and answers drug information questions for patients and healthcare professionals
• Screens for various drug interactions and contraindications
• Engages in relevant hospital and departmental committees

Duties

• Medication order approval and distribution
• Affirmation of appropriate dose, route, frequency, and duration of therapy
• Clinical profile review to assess safety and accuracy of medication services
• Patient medication profile review for drug-drug interactions, duplication of therapy, drug allergies, and appropriate regimens
• Coordination of patients' pharmaceutical needs with physicians, advanced practice providers, and nurses

Position Summary

This position coordinates all aspects of patients' pharmaceutical needs at The Ohio State University Wexner Medical Center. Key responsibilities include medication order verification, preparation oversight, clinical review, and coordination with healthcare providers.

Minimum Qualifications

• Bachelor's Degree in Pharmacy
• Registered Pharmacist license
• 0 years of relevant experience required
• 0-2 years of relevant experience preferred
