About the role

As a core member of the Pharmacy team, you will perform pharmaceutical care activities for customers and patients within the service area. This Per Diem role requires 1-2 shifts per week plus covering PTO, with a flexible schedule including on-call evenings and weekends as scheduled. Minimal travel (0-25%) may be required.

This position reports to the Pharmacy Manager and involves constant sitting, typing, occasional standing/walking, and the ability to push/pull and lift/carry 10-30 lbs.

Responsibilities

  • Evaluate and monitor drug therapy relative to the total needs of home care patients in collaboration with physicians, nurses, and other healthcare professionals.
  • Adhere to organization policies, procedures, and all applicable state and federal laws and regulations governing pharmacy practice.
  • Participate in quality assurance activities and adhere to accreditation standards.
  • Coordinate and facilitate new referrals and admissions through collaboration with nurses, customer service, and distribution.
  • Review, receive, and verify all new patient orders and document on respective order forms.
  • Assist physicians and healthcare professionals with:
    • Planning and implementing patient care in the home.
    • Setting therapeutic objectives for drug therapy.
    • Establishing parameters for monitoring the safety and efficacy of drug therapy.
    • Preparing regular progress reports.
  • Conduct and document initial and ongoing assessments of patient, drug, and disease information to identify real and potential drug-related problems.
  • Monitor outcomes of drug therapy by collecting, evaluating, and interpreting laboratory data, clinical data, and patient information.
  • Provide consultation and answer inquiries from patients, families, physicians, and other healthcare professionals regarding drugs and their use.
  • Maintain thorough and appropriate documentation in patient records of all clinical activities and communications.
  • Process and check all medications, supplies, and equipment prior to dispensing from the pharmacy.
  • Maintain clinical skills and knowledge of pharmacotherapy by reviewing literature and attending in-services, seminars, and conferences.
  • Compound sterile mixtures and non-mixtures, including parenteral nutrition, antibiotics, chemotherapy, hydration, and narcotic analgesics, using aseptic technique where permitted by state laws.
  • Ensure compounding/clean room areas and related equipment are cleaned, sanitized, and maintained in compliance with company policies, USP 797, and USP 800 standards.
  • Document all activities on appropriate logs.
  • Oversee Pharmacy technician functions as required by Pharmacy Board rules.

Requirements

  • Bachelor of Science in Pharmacy required; Doctor of Pharmacy preferred.
  • Registered Pharmacist License in state(s) of practice.
  • Minimum of two (2) years of recent experience in an acute-care setting or one (1) year of recent experience in home infusion therapy.
  • Experience with compounding infused drugs.
  • Proven record of evaluating and monitoring drug therapies for home care patients.
  • Good organizational and time-management skills.
  • Excellent verbal and written communication skills.
  • Participation and membership in professional societies and organizations is desirable.
