About the Role

The Pharmacist will report to the Pharmacy Manager and work in our City, State branch as a core member of the Pharmacy team. You will perform pharmaceutical care activities for customers and patients within the service area, with opportunities for continuous professional development and regular career progression discussions.

Schedule

Monday–Friday, 8:30 AM–5:00 PM (with the ability to work on-call evenings and weekends as scheduled). Minimal travel (0–25%) may be required.

Responsibilities

• Evaluates and monitors drug therapy relative to the total needs of the home care patient in collaboration with physicians, nurses, and other healthcare professionals.
• Adheres to organization policies and procedures and all applicable state and federal laws and regulations governing pharmacy practice.
• Participates in quality assurance activities and adheres to all appropriate standards of accreditation.
• Coordinates and facilitates new referrals and admissions through collaboration with nurses, customer service, and distribution.
• Reviews, receives, and verifies all new patient orders and documents on the respective order form.
• Assists physicians and other healthcare professionals with:
  • Planning and implementing patient care in the home
  • Setting therapeutic objectives for drug therapy
  • Establishing parameters for monitoring the safety and efficacy of drug therapy
  • Preparing regular progress reports
• Conducts and documents initial and ongoing assessment of specific patient, drug, and disease information to identify real and potential drug-related problems or needs.
• Monitors the outcomes of drug therapy by collecting, evaluating, and interpreting laboratory data, clinical data, and other pertinent patient information.
• Provides consultation and answers inquiries from patients, families, physicians, and other healthcare professionals regarding drugs and their use.
• Maintains thorough and appropriate documentation in the customer/patient record of all clinical activities and communication with patients, physicians, and other healthcare professionals.
• Processes and checks all medications, supplies, and equipment prior to dispensing from the pharmacy.
• Maintains clinical skills and knowledge of pharmacotherapy by reviewing literature and attending in-services, seminars, and conferences.
• Compounds sterile mixtures and non-mixtures, including parenteral nutrition, antibiotics, chemotherapy, hydration, and narcotic analgesics, using aseptic technique where state laws permit.
• Ensures compounding/clean room areas and related equipment are cleaned, sanitized, and maintained in compliance with company policies and procedures, and USP 797 and USP 800 standards.
• Documents all activities on appropriate logs.
• Oversees Pharmacy technician functions as required by Pharmacy Board rules.

Requirements

• Bachelor of Science in Pharmacy required; Doctor of Pharmacy preferred
• Registered Pharmacist License in state(s) of practice
• Minimum of two (2) years of recent experience in an acute-care setting or one (1) year of recent experience in home infusion therapy
• Experience with compounding infused drugs
• Proven record of evaluating and monitoring drug therapies for home care patients
• Good organizational and time-management skills
• Excellent verbal and written communication skills
• Flexible schedule with the ability to work on-call evenings and weekends as scheduled

Qualifications

• Participation and membership in professional societies and organizations is desirable

Physical Requirements

This role requires constant sitting and typing on a keyboard with fingers, and occasional standing and walking. The ability to push/pull and lift/carry 10–30 lbs is required.
