Responsibilities

• Initiate, develop, and implement an intermediate nursing plan of care, evaluating patient progress toward goals.
• Organize and provide direct patient care, including treatments, medication administration, and patient/family education.
• Modify the plan of treatment in response to changing patient status or physician orders.
• Assess patient needs and physical status at each skilled visit through health data access and patient interviews.
• Re-evaluate patient needs through physical reassessment, response to therapy, and supplemental physician orders.
• Obtain and clarify physician orders for plan of treatment revisions; inform physicians of significant changes in patient condition and provide written summaries.
• Provide training and mentorship to Infusion Nurse I’s to ensure patient safety and compliance.
• Deliver effective and safe teaching using a patient-centered approach to help patients and families achieve independence with prescribed therapy.
• Communicate effectively and timely with the clinical team, medical providers, patients, and families to facilitate continuity of care.
• Prepare clinical documentation in real-time during visits and submit to the nursing department to comply with billing timelines.
• Coordinate discharge planning, prepare discharge summaries, and report patient care needs, progress, and goals when transferring care.
• Demonstrate compliance with agency operations, policies, procedures, and professional standards, including local, state, and federal regulations.
• Maximize efficiency through the use of computers and automation technologies for validating orders and communicating care.
• Respond promptly to patient requests and initiate emergency procedures as necessary.
• Accept accountability for ethical and professional conduct, adhering to Option Care’s mission, values, and objectives.
• Safeguard patient confidentiality and proprietary information, including adherence to HIPAA regulations.
• Educate patients about Option Care’s scope of services and related financial obligations.
• Attend and complete required training, in-services, and continuing education to maintain competency and licensure.
• Provide oversight and coordination of paraprofessionals in the home setting; may act as Supervisor in the absence of the Nurse Manager.
• Participate in multidisciplinary team conferences and provide precepting, training, and mentoring to other nurses.
• Participate in nursing department on-call responsibilities.
• Perform other related duties as directed by supervisor.
• If applicable: Insert PICC/Midlines per physician order, troubleshoot/declot obstructed central line catheters, monitor PICC/Midline, and educate on infection prevention and routine care.

Requirements

• Active and unrestricted RN license in the state of practice.
• Minimum of 1 year of nursing experience.
• Current CPR certification.
• Demonstrated competency in patient care standards for safe service delivery and infusion skill sets.
• Required licensure to operate a motor vehicle in the state of practice with access to a vehicle for business travel and proof of liability insurance.
• Basic knowledge of computer operating systems and software applications for effective use of nursing technology tools.

Qualifications

• Bachelor of Science in Nursing from an accredited school preferred.
• 1-2 years of previous infusion nursing experience preferred.

Physical Demands

• Ability to lift up to 50 pounds, with a maximum lifting requirement of 75 pounds.
• Exerting up to 20 pounds of force occasionally or up to 10 pounds frequently.
• Physical demands may include walking, standing, crouching, kneeling, turning, pivoting, balancing, stooping, reaching overhead, grasping, pushing, pulling, lifting, and carrying.
• Fine motor skills and visual acuity, including close and distance vision, color and peripheral vision, depth perception, and ability to adjust focus.
• Likelihood of occupational exposure to blood, body fluids, and other potentially infectious materials.
• Possible exposure to hazardous substances, adverse weather, animals, secondhand smoke/vape, and unpredictable home environments.
• Safety requirements include closed-toe shoes, facemask, goggles, gown, gloves, and a functioning communication device.

Travel

Willing to travel up to 100% of the time for business purposes.

Pay

Salary to be determined by the applicant's education, experience, knowledge, skills, and abilities, as well as internal equity and market alignment. Pay range is $34.95–$58.25 per hour.
