About the Role
The Infusion Nurse provides primary patient care for specifically assigned patients to include assessments, care planning, evaluation, and education of patients receiving infusion therapy and/or other nursing services in accordance with care matrices and best practices in the home or alternate site setting.
Responsibilities
- Provide direct care for all assigned patients including initiation and maintenance of communications with appropriate care team members (pharmacists, prescribers, dietitians, home patient representatives/care partner, and community resource staff).
- Evaluate and educate patient/care partner to safely administer home infusion therapy and provide additional instruction as indicated.
- Evaluate the home environment where care is to be provided and identify interventions to render the home care environment safe and appropriate for prescribed care.
- Develop the plan of care in conjunction with other team members and evaluate response to therapy with each encounter.
- Maintain caseload scheduling via company-approved platform per company policy and communicate scheduling changes to appropriate team members.
- Coordinate care via approved company platform and participate in the nurse on-call rotation to ensure continuous nursing support.
- Document all communications at time of service as appropriate per Soleo policy.
- Work in conjunction with appropriate departments to ensure all required patient documents are completed and returned to the branch office within an acceptable timeframe.
- Ensure timely submission of plan of treatments as required by state guidelines.
- Provide administrative oversight, including monitoring workflows, ensuring compliance with policies, and managing resources efficiently.
- Respond promptly to emergencies to ensure rapid and effective resolution.
Requirements
- RN Degree (BSN preferred).
- Advanced certification in specialty areas of practice (e.g., CRNI, ONS, IgRN) preferred but not required.
- Minimum 3 years nursing experience with a background in infusion therapy preferred.
- Current single-state RN license for Alabama in good standing.
- Experience in ICU and/or Cardiac Care preferred.
- Proficient with Microsoft Outlook and Microsoft Office Suite; preferred experience using CPR+, Insight, Concur, AlayaCare, and Paylocity.
- Must be knowledgeable of IV access devices, including IV lines, ports, and administration of infusion medications.
- Must be able to drive your own properly registered/insured vehicle up to 2 hours one direction to see patients as needed.
- Valid state-issued driver’s license with no significant restrictions.
Schedule
- Per Diem, 20 hours per week, variable schedule.
