About the role
We are seeking a Board-Certified or Board-Eligible Infectious Disease Physician (MD/DO) to deliver on-site infectious disease consultative services in skilled nursing and long-term care settings.
Responsibilities
- Manage complex infections, oversee antimicrobial therapy, and collaborate with interdisciplinary teams and Advanced Practice Providers.
- Provide infectious disease consultations for SNF/LTC residents.
- Diagnose and manage complex infections including pneumonia, UTIs, bacteremia, osteomyelitis, wound infections, HIV, and viral hepatitis.
- Direct antimicrobial therapy including IV and oral regimens.
- Order, interpret, and act upon cultures, labs, imaging, and diagnostic studies.
- Lead and support antimicrobial stewardship initiatives.
- Supervise and collaborate with Nurse Practitioners and facility clinical staff.
- Advise on infection prevention, isolation, and regulatory compliance.
- Ensure accurate, timely EMR documentation in accordance with CMS and state requirements.
Requirements
- MD or DO degree from an accredited medical school.
- Completion of an accredited Internal Medicine residency.
- Fellowship training in Infectious Disease preferred.
- Active and unrestricted New York State medical license.
- DEA registration.
- Board Eligible or Board Certified in Infectious Disease.
- Experience in SNF/LTC or post-acute care preferred.
