About the role

Essen Health Care is a growing community healthcare network providing high quality, compassionate, and accessible medical care to vulnerable and under-served residents of New York State. Guided by a population health model of care, Essen offers services in primary & specialty offices, urgent care centers, and nursing homes, with house calls for home-bound patients. All clinical services are also provided via telehealth.

Responsibilities

  • Review the patient's medical records, including X-rays, blood work, and other lab tests.
  • Evaluate patients through conversations, interviews, and observations.
  • Administer patient tests or request testing/lab work; analyze patient data and results.
  • Collaborate with patient's physicians and other medical staff to determine treatment.
  • Set patient's goals and discuss treatment plans.
  • Establish timelines, tasks, and medical needs of the patient.
  • Analyze the best medications or therapies to help the patient reach their goal.
  • Prescribe medications and monitor patient intake.
  • Document patient records and progress, including symptoms, medications, and treatments.
  • Counsel the patient and listen to their reactions to treatment.
  • Provide the patient with emotional tools and methods for recovery.
  • Educate the patient, patient's family, significant others, and relevant parties on the patient's progress, treatment, and future goals.

Requirements

The successful candidate must have:

  • A Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO).
  • Board Certification or Board Eligibility in Internal Medicine or Infectious Diseases.
  • Successful completion of an approved residency training program.
  • Active License in New York State.
  • Current Drug Enforcement Agency (DEA) and Department of Public Safety (DPS) registration for prescription writing.
  • Bilingual (Spanish/English) is preferred but not required.

Qualifications
