About the role
Essen Health Care is a growing community healthcare network providing high quality, compassionate, and accessible medical care to vulnerable and under-served residents of New York State. Guided by a population health model of care, Essen offers services in primary & specialty offices, urgent care centers, and nursing homes, with house calls for home-bound patients. All clinical services are also provided via telehealth.
Responsibilities
- Review the patient's medical records, including X-rays, blood work, and other lab tests.
- Evaluate patients through conversations, interviews, and observations.
- Administer patient tests or request testing/lab work; analyze patient data and results.
- Collaborate with patient's physicians and other medical staff to determine treatment.
- Set patient's goals and discuss treatment plans.
- Establish timelines, tasks, and medical needs of the patient.
- Analyze the best medications or therapies to help the patient reach their goal.
- Prescribe medications and monitor patient intake.
- Document patient records and progress, including symptoms, medications, and treatments.
- Counsel the patient and listen to their reactions to treatment.
- Provide the patient with emotional tools and methods for recovery.
- Educate the patient, patient's family, significant others, and relevant parties on the patient's progress, treatment, and future goals.
Requirements
The successful candidate must have:
- A Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO).
- Board Certification or Board Eligibility in Internal Medicine or Infectious Diseases.
- Successful completion of an approved residency training program.
- Active License in New York State.
- Current Drug Enforcement Agency (DEA) and Department of Public Safety (DPS) registration for prescription writing.
- Bilingual (Spanish/English) is preferred but not required.
