Responsibilities
- Develop and implement a facility-wide Antibiotic Stewardship Program based on nationally recognized guidelines.
- Monitor and improve the use of antimicrobials through documentation of program activities.
- Communicate and collaborate with medical staff, nursing, laboratory, pharmacy, and hospital leadership on antimicrobial use initiatives.
- Train and educate hospital personnel and staff, including medical staff, on the practical applications of antibiotic stewardship guidelines, policies, and procedures.
- Provide pharmaceutical care by reviewing providers' orders for drug appropriateness, dosage, route, and potential interactions.
- Assist nurses and providers with medications and medication-related issues.
Qualifications
- Post Graduate Degree (PHARM D Required)
- Pharmacist License
- BLS
- PGY 1 Pharmacy Residency and/or BCPS Certification within 3 years from date of hire.
