About the role

Venture Community Services provides support to individuals facing diverse challenges through a compassionate and dedicated workforce using innovative practices that encourage independence, empowerment, and opportunity. The Individual Supports Caseworker delivers direct training to individuals in compliance with Agency policy and procedures, focusing on community living skills for developmentally delayed persons to help them remain in their current home environment.

Responsibilities

  • Provide home visits to assess new referrals and implement Individual Skills Support strategies weekly.
  • Maintain daily activity statistics and documentation as required by Agency and contract policy and procedure.
  • Represent the department in multi-disciplinary meetings for planning and review of Individual Support Plans.
  • Participate in all required staff and Agency meetings.
  • Provide direct individual training in the community of community living skills as defined by program contract.
  • Attend workshops and other continued education programs.
  • Report to the Program Director of Shared Living, AFC & Individual Supports for accountability of scheduled hours.
  • Compose reports as required by Agency and contract policy and procedures.
  • Maintain confidentiality and a positive demeanor while managing multiple priorities.
  • Maintain confidential and budgetary records in accordance with funds management protocols.
  • Communicate with Program Director regarding service delivery reports and contract hour goals progress.
  • Provide recommendations to your supervisor regarding ways to improve current systems.
  • Observe all Agency policies and procedures.
  • Maintain current CPR and First Aid Certifications.
  • Presence in administrative office as needed.
  • Other duties as assigned.

Requirements

  • Bachelor's Degree in education, psychology, rehabilitation, or related field and one year of professional experience preferred; OR 60 credits toward a Bachelor's Degree with continuing study and 3 years’ experience in special education.
  • Must be 21+ years old, have a High School Diploma or GED, a valid driver’s license, and daily use of a car on site.
  • Must be available by telephone for daily contact.
  • Must possess effective verbal and written communication skills in fluent English.
  • Ability to establish professional rapport with developmentally disabled individuals, their families, co-workers, and service providers.
  • Impeccable time management and organizational skills.
  • Ability to make and keep appointments in the community.
  • Ability to analyze tasks for step-by-step instruction.
  • Strong interpersonal and communication skills, and the ability to work with a wide range of employees.
  • Organized, dependable, and even temperament; good phone manner.
  • Working knowledge of Microsoft Office and office equipment.
  • Must be able to bend, lift, stoop, reach, and file.

Pay

$23 per hour. Paid training provided by Venture. Sign-on bonus: $1000 for Full Time and $500 for Part Time employees.

Schedule

Monday-Friday, full time: 40 hours to fit the needs of the program.
