Minimum Qualifications

For a Lease Agreement: - Doctor of Optometry (OD) from an accredited optometry school - Valid state optometry license - Ophthalmologist (MD or DO) from an accredited school - Valid state ophthalmology license - General Liability Certificate - Professional Liability Insurance Certificate - W-9
