About the Role

Independent Optometrist – Sam’s Club. Lease space inside a Sam’s Club and lead patient care for eye, vision, and general health. As an independent lease holder you have full autonomy over your practice.

Responsibilities

• Establish your days of practice
• Set your hours of operation
• Control your patient schedule
• Determine your fee schedule
• Retain 100% of exam and professional fees
• Optionally lease equipment/instrumentation for a fair market value
• Optionally obtain administrative and/or Opt‑Tech support services (if state allows)

Minimum Qualifications

• Doctor of Optometry (OD) from an accredited optometry school
• Valid state optometry license
• Ophthalmologist (MD or DO) from an accredited school
• Valid state ophthalmology license
• General Liability Certificate
• Professional Liability Insurance Certificate
• W‑9
• ME & VA license agreements are with Gumberg Asset Management Company
