Position Summary
Launch your services in Sam's Club! As an Independent Optometrist leasing space inside our stores, you will take a leading role in patient care with respect to eye and vision care as well as general health and wellbeing.

- Take a leading role in establishing your days of practice.
- Take a leading role in establishing your hours of operation.
- Take a leading role in controlling your patient schedule.
- Take a leading role in setting your fee schedule.
- Take a leading role in keeping 100% of exam/professional fees.
Minimum Qualifications
- Doctor of Optometry (OD) from an accredited optometry school.
- Valid state optometry license.
- Ophthalmologist (MD or DO) from an accredited school.
- Valid state ophthalmology license.
- Advanced Equipment/instrumentation and Administrative and/or Opt-Tech support services if state allows.
- Minimum Qualifications for a Lease Agreement with Gumberg Asset Management Company.
