About the Role
Ascend Healthcare is seeking a Licensed Therapist or Counselor (LPC, LCSW, LMFT) to provide counseling and therapy treatment to patients remotely via telemedicine. Services will be provided to patients of Community Health Center partners throughout Arizona. The clinician must hold an active license in their state of primary residence and either hold or be willing to obtain an Arizona license.
Key Areas of Ownership
- Provides patient care using evidence-based practices to meet clinical needs, including CBT, DBT, Solution-Focused Therapy, Psychoeducation (Symptom Recognition), Trauma Therapy, Addiction Therapy, Acceptance and Commitment Therapy, and Bereavement/Grief Counseling
- Implements treatment modalities in both individual and group therapy settings with a focus on engaging patients in their treatment plan
- Reviews and signs patient clinical psychosocial assessments, diagnoses, treatment plans, and consents in a timely manner
- Maintains clinic records, including documentation of all services and patient activities performed as part of the service delivery process
- Ensures documentation complies with best clinical practices, contracts, regulatory requirements, and other legal standards
- Conducts crisis intervention for individuals and families based on the individual's safety plan
- Participates in regular case planning with the multidisciplinary team to ensure clinically appropriate patient disposition
- Participates in individual and group supervision as required
- Participates in ongoing education, including in-services, training, and other activities to maintain and improve competency
- Demonstrates competency through post-testing, skill observation, and performance assessment by direct supervisor
- Provides case management services to promote recovery and community reintegration as needed
- Performs other duties as assigned
Qualifications
- Active, unrestricted mental health license in state of primary residence and state of medical services being provided (Arizona)
- Comfortable working in a remote environment
- Willingness to work evening hours

Physical Requirements
This is a fully remote position but may require in-person attendance as company needs arise.
- Prolonged Sitting: Ability to sit for extended periods during working hours
- Manual Dexterity: Good hand-eye coordination and manual dexterity for using a computer keyboard, mouse, and other office equipment
- Visual Requirements: Adequate vision for reading computer screens and documents
- Communication: Clear verbal and written communication skills for virtual meetings and correspondence
- Hearing: Sufficient hearing ability for participating in phone calls or virtual meetings
- Work Environment: Access to a quiet, dedicated workspace free from distractions with reliable internet connectivity

Pay
- Job Type: Full-time
- Starting Minimum Salary: $80,000-$90,000 with primary residence in AZ

- 5, 8-hour or 4, 10-hour shifts
