Independent Living Specialist - East Harlem Safe Haven

Responsibilities

• Provide comprehensive case management services to identified chronic homeless individuals
• Meet with clients on a regular basis both in the field and in the office
• Ensure that all clinical files and reports are complete and up to date
• Conduct comprehensive assessments to identify barriers to housing
• Assist clients to overcome housing barriers and secure entitlements
• Develop and work with clients to progress on individualized housing plans
• Build a network of support services for clients
• Prepare clients for housing interviews and arrange housing appointments
• Participate in case conferences as necessary
• Liaison with clinical staff in other BRC or outside programs with regard to common clients
• Provide emergency first aid/CPR or overdose prevention protocol assistance when needed

Qualifications

• High School Diploma/GED required; bachelor's degree preferred and master's ideal
• Bilingual Spanish strongly preferred, but not required
• Excellent communication and writing skills and computer proficiency required
• Experience working in residential treatment and with the MICA and homeless populations
• Knowledge of HRA and NY/NY application systems a plus
• CPR training certification or willingness to take training class in CPR
• Vaccination preferred but not required

Full-time, 37.5 hours per week, Sunday-Thursday 7:30am-4pm
