Independent Living Coordinator

Responsibilities

  • Initiate and maintain ongoing contact with youth referred through Huntingdon County Children & Youth Services
  • Conduct approved assessments and assist youth in developing Independent Living Goal Plans within the first month of service
  • Complete quarterly reviews of Independent Living Plans with participating youth
  • Schedule, plan, and facilitate group activities based on youth needs and availability
  • Collaborate with families, county agencies, and community partners to support service goals
  • Provide guidance, support, and resource coordination to youth and families
  • Prepare and maintain accurate, timely, and compliant documentation, including case notes
  • Ensure compliance with agency policies, state regulations, and program requirements
  • Attend meetings, case reviews, and court proceedings as needed
  • Provide professional court testimony when required

Requirements

  • Bachelor’s Degree in related field; OR
  • Bachelor’s Degree in an unrelated field with a minimum of 12 credits in a Human Services related course; i.e. psychology, sociology, counseling etc.
  • Or Associate's Degree with 2 years' experience working with children, youth, and families
  • A valid Pennsylvania Driver’s license with good driving record

  • Schedule is typically 8am to 4:30pm, M-F (is dependent on the families’ needs)
  • Competitive pay rate! $18.00 to $19.00 per hour based on education and experience
