About the role

The Support Services functions at S:US are carried out by a team of professionals with differing backgrounds who share deeply in the S:US mission. As an Incident Management Specialist, you will join a dynamic department that protects the organization as well as the people we serve. Your role will ensure that all incidents are reported in compliance with regulatory requirements and that all incident management activities are completed accurately and timely. You will also protect the health and safety of the individuals we serve by ensuring protective measures are put into place to prevent the reoccurrence of incidents. This position requires cross-departmental communication and the ability to operate in a fast-paced environment. You will abide by incident management regulations for various regulatory agencies to ensure compliance with Federal, State, and local laws, while adhering to the S:US mission, core values, and code of ethics.

Responsibilities

  • Incident Management Activities
    • Serve as the primary QA contact for all program staff and external oversight bodies regarding incident management reporting and processes.
    • Interpret and apply incident management regulations, providing technical support to staff.
    • Triage reports of all incidents to ensure protective measures are in place for consumer safety, including attending daily triage meetings.
    • Review all notifications, preliminary investigations, occurrence reports, and other supporting documentation; follow up with program staff on missing pertinent information to ensure compliance.
    • Serve as the Quality Assurance co-chair in monthly Incident Review Committee (IRC) meetings.
    • Complete minutes and follow-ups for IRC meetings, submitting them to program management and appropriate staff.
    • Review all incidents to ensure IRC minutes and follow-ups are completed timely and submitted for review.
  • Trainings
    • Conduct Incident Management trainings for all staff levels.
    • Develop and update training materials for Incident Management trainings.
  • Reporting/Data Analysis
    • Maintain incident management reporting logs with data to support incident trends.
    • Manage data (including submission of corrective action plans) in required state computerized incident management systems (e.g., IRMA and NIMRS).
    • Draft monthly incident trend reports, analyzing trends pertaining to consumer incidents, and request appropriate corrective actions.
    • Conduct post-incident analysis to ensure accurate root cause identification and track preventive actions.
  • Communication/Networking
    • Provide incident updates to stakeholders and leadership as required (verbal and/or written).
    • Request additional information from external sources to thoroughly review incidents.
    • Provide required notifications and updates on all incidents to established department heads.
    • Coordinate with program leadership and HR to ensure swift resolution of incidents.

    • Communication: Demonstrate effective communication and good interpersonal skills when interacting with individuals served, co-workers, and management. Maintain open communication with your supervisor regarding assignment completion and accuracy.
    • Quality of Work: Ensure assignments are accurate, complete, timely, and comply with agency and department policies. Work independently to overcome challenges and meet performance objectives.
    • Integrity/Compliance: Uphold personal and professional ethics, respecting the organization’s values and the culture, beliefs, and abilities of individuals served. Ensure compliance with HIPAA and other regulatory requirements.

    • Self-Motivated/Flexible: Demonstrate initiative, maintain productivity, and balance competing priorities with minimal supervision. Adjust to changes in routine and assignments while remaining productive.
    • Job Knowledge: Demonstrate skills and knowledge relevant to job functions, applying current best practices and staying aware of developments in your specialty area. Exhibit expertise in relevant regulatory requirements.
    • Writing: Convey ideas and facts in writing using clear, concise, and logical language appropriate for the intended audience. Use correct vocabulary, spelling, grammar, and punctuation.

Requirements

  • Bachelor’s Degree in a related field, with a minimum of two (2) years of experience in the healthcare or non-profit field.
  • Willingness to:
    • Travel extensively in all boroughs.
    • Work a flexible schedule to meet department needs.
    • Be on call during non-office hours, evenings, weekends, and holidays when necessary.

Qualifications

  • Preferred:
    • Master’s Degree.
    • Incident Management experience.
  • In lieu of a Bachelor’s Degree, a High School Diploma or equivalent with a minimum of seven (7) years of experience in the healthcare or non-profit field.

Min USD $65,000.00/Yr. Max USD $80,000.00/Yr.

  • Flexible schedule to meet department needs.
  • On-call duties during non-office hours, evenings, weekends, and holidays when necessary.
